Restructuring Health Insurance Markets Elliot K. Wicks January 30, 2009 Abstract: Many states-and some federal legislators-are considering major policy changes to make health insurance more affordable and to increase the number of people who have health coverage. Often these policies involve changes in the structure of health insurance markets, such as establishing risk pools or insurance exchanges for certain populations and altering the rules for selling coverage in the individual and small-group markets. This paper examines six specific structural changes that might be made to expand coverage, with special focus on the administrative issues that must be addressed. The six are: changes in rating rules, high risk pools, standard benefit plans, reinsurance, Section 125 plans, and insurance exchanges.
